We have different teams set up in conjunction with providing better service. To take better care of patients and their needs, we have a Front Office team, Billing team, Nursing team, and an Administration team. The Referral department makes it easy for the patient to choose a Specialty doctor or a specialty care center, schedule an appointment, and do follow-up with the results. Patients can talk about their billing questions/concerns directly to the billing staff.

EKG, Cardiac blood workup, Exercise ECG treadmill stress testing
In addition, our newest feature 'T-Wave Alternans stress testing' measures your heartbeat at "microvolt" level. It is a highly advanced state-of-the-art screening for heart disease.

X-RAY CAPABILITY
We do x-rays in-house. That helps in diagnosing deformity, pneumonia, fractures, and other abnormalities. X-rays for chest, arm, spine, abdomen, extremities, neck, knees, etc. are performed in-house for the convenience of our patients.

ULTRASOUNDS
We do ultrasound procedures for pelvic, abdomen, kidney, breast etc. We perform Cartoid ultrasound that checks for blocked arteries, 2-D Echo for valve functioning and overall efficacy of heart.

BONE DENSITY DEXASCAN OSTEOPOROSIS SCREENING
This is a very detailed screening that measures the amount of calcium in specific regions of the bones. This is the primary screening tool for osteoporosis.

COMPLETE FAMILY WELLNESS PROGRAMS
This is a great tool to track the state of your health throughout your lifetime from childhood to adult. It includes Flu Vaccines, Immunizations, Vision and Hearing Screenings, Prostate Exams, Gynecological Exams, and Physicals.

PREVENTATIVE MEDICINE AND EDUCATION PROGRAMS
It includes management of Diabetes, Thyroid, Cholesterol, and High Blood Pressure. We have annual physicals, general physicals, healthy heart package, sports and wellness physicals.

NO INSURANCE.........NO PROBLEM

Family Doctors of Green Valley offer the lowest cash prices in the Valley. We offer special cash prices for office visits and other wellness procedures to persons without insurance. We offer low cash prices for ancillary services like EKG's, ultrasounds, breathing treatments, wellness physicals, IV's. We accept any mode of payment; you can use cash, ATM's, credit cards, or set up payment plans with our billing department.

Shortly, we will be setting up an in-house Medical Savings Plan, which will be a great program for all of our patients. It is once-a-month payable pre-paid card that allows you to pay for office visits and procedures done at our office. The unused portion can be carried over for your future visits at any time.

Family Doctors of Green Valley believes in offering Preventative/Wellness measures, which contributes for well-being of an individual for the rest of their life.

Annual Physicals:
We do Annual Physicals that are helpful in evaluating patient's health, especially for patients above 30 yrs. of age. According to your convenience, the tests are scheduled and have trained personnel conducting them. Our providers let you know about the effective measures to be taken. We store your results in a Personalized Physicals booklet categorized by the type of test done. Each year, the tests are done and can be stored chronologically in your physical booklets. The pattern of your tests can be studied by referring to your physical booklet.

General Physicals:
General Physicals will be conducted as and when it is required or provider feels it is necessary. General Physicals encompass the following:

1. Initial Visit with consultation, medication review, physical examination
2. Laboratory studies (cholesterol, thyroid, blood counts, kidney, liver, electrolytes)
3. Radiological Tests such as chest x-ray, mammogram
4. Stress treadmill and EKG
5. Hearing and Vision screening
6. Prostate exam for men, gynecological exam for women
7. Dexa Scan (screening for osteoporosis)
8. Referral for Colonoscopy (screening for colon cancer)

Healthy Heart Packages:
This is a more comprehensive exam, which includes detailed study that is designed to identify risks for future cardiovascular events such as stroke or heart attack. Testing includes the following:

1. VAP panel for detailed cholesterol report including homocysteine, and Cardiac CRP levels.
2. Stress treadmill and EKG (screens for rhythm abnormalities, early heart disease)
3. Carotid Ultrasound (evaluates carotid artery blood flow)
4. T-Wave Alternan (identifies sudden risks of heart disease based on cardiac rhythm abnormalities)
5. Echocardiogram (evaluates heart function, valve disorders, chamber size)

For uninsured patients, we offer a comprehensive panel for $75, which includes the following:

1. Fasting Cholesterol panel (includes total cholesterol, HDL, LDL, triglycerides)
2. Thyroid screening
3. Anemia/Blood Count testing

For female patients, pap smears/gynecological exams are at affordable prices. The basic "pap" smear with examination is offered at $145 for new patients and $135 for established patients.

1. Do we need to schedule an appointment in advance?
We appreciate and welcome walk-ins if you would like to be seen by any one of our doctors. If you would like to see a particular doctor, then you need to schedule an appointment in advanced.

2. Are there any charges for cancelling the appointment?
Patient will not be charged for cancelling the appointment but we would appreciate a 24 hr. notice for cancellation. This will help us to keep the office running efficiently for everyone.

3. How can we get prescriptions from the office?
A patient can receive prescriptions when he conveys so to the nurse or doctor. The turn-around time for prescription refills is 48-72 hours. However, prescriptions for narcotics will not be given on the phone unless you have a scheduled an appointment and were seen by a doctor .

4. What is the procedure to have blood tests or certain procedures done?
We do most of the blood work and procedures in our office, but you need to schedule an appointment for the blood draws, ultrasounds, dexascans, stress test etc. You can call the front office for more details.

5. What if a patient has a deductible?
If a patient has a deductible, he/she has to pay for the office visit in cash up front. They will be charged $85 for an office visit and accordingly for other procedures done which will be applied to the deductible.

6. What forms of payment are accepted?
We accept payment in cash, check, credit card, and ATM debit card with Visa or Mastercard logo. We also accept American Express and Discover as forms of payment. You also have an opportunity to set up a payment plan with our billing department where you can receive detailed information.

7. What Medical Insurances are accepted?
We accept most of the insurances that are PPO's. You can contact us to find out if your specific insurance is accepted or not.

8. What do we need to bring at the time of visit?
You need to carry your health insurance card and picture identity card. If you are having the appointment concerning a medical report, you need to get one if it is with you.

9. Where are you located?
Our clinic is conveniently located at the Northwest corner of Eastern and Wigwam. We are two blocks north of the 215 Freeway. Our address is 8475 S. Eastern Avenue, Suite 204 , Las Vegas , NV 89123.

It is the purpose of Family Doctors of Green Valley that all physicians and staff preserve the integrity and the confidentiality of all Protected Health Information (PHI) pertaining to patients by creating privacy-conscience business practices. Those practices should ensure the internal protection of medical records, employee privacy, training and education, creation of a mechanism for addressing patient privacy complaints, and the designation of Family Doctors of Green Valley's HIPAA Privacy Officers.

Patients should not fear about providing Family Doctors of Green Valley the information that is needed for the purposes of treatment, payment and healthcare operations (TPO).

Family Doctors of Green Valley physicians and staff will:

• Adhere to the standards set forth in the Notice of Privacy Practices.
• Collect, use and disclose PHI only in conformance with State and Federal Laws
  current patient covenants and/or authorizations.
• Not use or disclose PHI for uses outside the practice's TPO, such as marketing, employment, life insurance applications, etc. without the patient's authorization.
• Use and disclose PHI to remind patients of their appointments only within their consent.
• Recognize that PHI collected about patients must be accurate, timely, complete and available when needed.
• Implement reasonable measures to protect the integrity of all PHI maintained about patients.
• Recognize that patients have a right to privacy. Physicians and staff shall respect the patient's individual dignity and privacy to the extent consistent with providing the highest quality medical care possible and with the efficient administration of the facility.
• Act as responsible information stewards and treat all PHI as sensitive and confidential in accordance with professional ethics, accreditation standards and legal requirements.
• Not disclose PHI data unless the patient (or authorized representative) has consented to or authorized the release, or law otherwise authorizes the release.
• Recognized that, although Family Doctors of Green Valley owns the medical record, the patient has the right to inspect and obtain a copy of his/her PHI. Patients also have a right to request an amendment to his/her medical record if he/she believes his/her information to be inaccurate or incomplete. Family Doctors of Green Valley physicians and staff shall:
• Permit patient access to their medical records when their written requests are approved by Family Doctors of Green Valley. If a request is denied, the patient must be informed of the denial and that a review of the denial may be requested by the patient. An on-site healthcare professional will review any patient appeals.
• Provide patients with an opportunity to request the correction of inaccurate or incomplete PHI in their medical records in accordance with the law and professional standards.
• Maintain a list of all disclosures of PHI for purposes other than TPO for each patient. Family Doctors of Green Valley will provide this list to patients upon written request.
• Adhere to any restrictions concerning the use or disclosure of the PHI that patients have requested and have been approved by Family Doctors of Green Valley.

All Physicians and staff of Family Doctors of Green Valley will adhere to this policy and all as above.

Family Doctors of Green Valley reserves the right to change this privacy policy in the future as changes in the practice's dynamics and any changes in the HIPAA rules and regulations occur. Any revisions will be made available to patients upon request.
